Confidentiality Policy

Protected Health Information Confidentiality Policy:

The Gordon State College Health Services adheres to all federal and state laws that apply to the health care of students. All medical records are strictly confidential and private. Student Employees working in the Gordon State College Health Center will adhere to all of the terms and agreements in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Employees are required to complete HIPAA compliance training with the nurse practitioner and will receive a certificate after successful completion.

Gordon State College Health Services is required by law to maintain the privacy of medical information and must provide the student with this information about privacy practices. With some exceptions, the Health Services may not disclose any information that is necessary to accomplish the purpose or use of the disclosure.

Uses and disclosures without authorization:

The following categories describe the ways that Gordon State College is permitted to use and disclose student's medical information without authorization from the student:

  • As required by law: Gordon State College is obligated by law to disclose medical information pertaining: (a) victims of abuse, neglect, domestic violence, or rape, (b) to report reactions in the manufacture of medications or problems with surgical supplies, or (c) to report a recall of a product that the student may be using.
  • To avert a serious health or safety threat: Medical information may be used and disclosed when necessary to prevent a serious threat to student's health and safety and/or the health and safety of the public.
  • For emergent care and treatment: With emergent care. Medical information may be required for continuity of care of a student's status for medical treatment. Medical information may be disclosed to hospitals, physicans, nurses, and other health care personnel who provide those health care services and treatments.

Uses and Disclosures Requiring Authorizations:

Any other use and/or disclosure that is not covered by this notice. Authorization by the student is required to release any information with written permission of medical information.

Students' Rights Regarding their Medical Information:

Students have the right to inspect and receive a copy of medical information that may be used to make decisions about their care and treatment. Students have the right to restrict or limit the medical information used or disclosed for treatment or referral (exception: legally required or allowed disclosures). Students have a right to a paper copy of this notice with a signed release of information.
