Please be sure to use the form with the correct practicum number listed at the top of the page. The video form is for senior teacher candidates only. This form should only be used for the video observation during practicum III or student teaching.

Please fill out the form after each observation. Take the time to discuss the observation with your teacher candidate and email them a copy of the form. The Gordon Teacher Education Office will need a copy of each form with your original signature. These can be turned in as a batch at the end of each semester.
